I asymptomatic, or minimal headache and neck stiffness 15 absent
II severe headache, neck stiffness, cranial nerve palsy (optional) 13–14 absent
III drowsiness, minimal neurologic deficit 13–14 absent
IV stupor, severe neurologic deficit (hemiparesis), vegetative disturbances 7–12 present or absent
V coma, signs of decerebration 3–6 present or absent
