Overview of the PostStroke-Manager concept with combined digital support and personal assistance. Using a tablet with a pre-installed app, 10 components will be available for the patient that cover different aspects of post stroke management. Personal assistance will be given by a nurse with special training in case management and typical issues arising in acute and later stages of stroke.
