Conceptualization of the PostStroke-Manager with a central position of the patient, equipped with a tablet and mobile devices for registration of physical activity, blood pressure, and electrocardiographic signals. Using WLAN or SIM cards, the tablet will be connected to a server allowing interactions with the primary care physician, the stroke pilot as well as specialists if indicated.
